Senior home health care
Qualifications:
The field nurse position minimally requires current New Jersey RN license in good standing.
The company would prefer Field Nurse to have minimum of one year professional experience as an RN in community health and/or home health, with progressive nursing responsibilities.
Accountability:
Reports to the Director of Nursing. This position consults with the Care Coordinator.
The field nurse's primary area of accountability is for the supervision of patient care and supervision of the home health care personnel within the guidelines set forth in local, state and federal law as well as the agency's standards.
Position/Focus - Areas of Responsibility:
The field nurse's duties include review of case intake to determine assignment of appropriate level of care, contact with practitioners to establish appropriate patient care plans and assure timely receipt of practitioner's orders, to conduct bi-monthly in-home visits to assess patient needs, revise care plan, update practitioner orders, review charting kept by field personnel and to provide supervision of all personnel in the field to orient them to their assignments as well as evaluate their performance, and maintain accurate patient care records.
The field nurse monitors the skills and services of caregivers, monitors the changing needs of the patient and family and assures a safe working environment for all caregivers. He/she is also responsible for accurate, timely recordkeeping and weekly review/maintenance for both documentation and patient records.
Additional job responsibilities include:
- teaching inservice education classes for field personnel
- performing personnel conduct and performance evaluation reviews-performing "on-call" that may require making home visits during non-office hours.
