Affinity Home Health Care Services has been providing home health care and related home care services since 2005. The Company is licensed by the New Jersey Department of Consumer Affairs. Affinity is also accredited by the Commission on Accreditation for Home Care (“CAHC”).

We are devoted to identifying and serving the needs of our patients in consultation with their attending physicians. We put our clients first; we make sure their needs are met on time and in an orderly manner.

We only use qualified caregivers who are insured and bonded:

We only recruit, hire and place qualified caregivers on our home health care and other home care cases. All of our home health care workers are credentialed to provide the necessary services for our patients. Our caregivers are all insured and bonded for extra protection of our clients’ interests. We take great pride on putting the “right” caregiver on each case.

Quality Assurance and Quality Improvement Program:

Affinity has developed and implemented an educational and supervisory program to ensure that our patients and other clients receive the very best services. We strive to provide high quality services; we also pursue a process of perpetual self improvement. We have implemented the following measures to achieve these desired goals:

• Use of only qualified caregivers
• Our caregivers are compassionate, sensitive and devoted to our clients’ needs
• Criminal back ground checks on caregivers
• Random drug testing of caregivers
• Careful screening of all caregivers in terms of skills, experience and psychological makeup
• Initial orientation and on-going education of all caregivers
• Clinical supervision of all caregivers by a Registered Nurse
• Services provided consistent with the Doctor’s Orders and applicable laws and regulations
• All caregivers are insured and bonded
• All caregivers have been tested to ensure their competency and understanding
