Care health home services
Home health care services are provided by certified and trained caregivers/home health aides. The individual care plan is coordinated after the assessment interview and is reviewed and supervised by the visiting field nurse.
Scope of Personal Care Services:
- Bathing (tub, shower, sponge bath, bed bath)
- Grooming (wash and brush hair, file nails, shave)
- Mouth Care (teeth, mouth, dentures)
- Dressing (upper and lower body, undergarments, socks, stockings, shoes)
- Skin Care (apply lotion, light back rub, observe condition of skin)
- Toileting (toilet, commode, bed pan, urinal, diaper, colostomy, catheter)
- Help with Transfer (bed, chair, toilet, wheel chair, Hoyer Lift, pivot, guard, assist)
- Assist with Walking (person assist, guard, walker, cane, wheel chair)
- Perform exercise as prescribed (Passive and Active Range of Motion)
Scope of the Companion Care Services:
- Meal preparation (follow medically prescribed diet, assist and supervise during meal as necessary)
- Medications (remind, assist, open bottles)
- Companionship, emotional support, encourage conversation, reality orientation
- Light housekeeping (clean bedroom, bathroom and kitchen after client's use)
- Make bed, change and launder linens
- Do laundry (for client only)
- Shopping (food and other items)
